Principal Software Engineer (AI / Healthcare) — Europe / Remote

Job Description

Madrid, Berlin, London — Remote | €100,000 – €200,000 + equity
Full-time | Senior / Principal Level

About the Role

Our client is a stealth healthcare AI startup backed by a top-tier global venture capital firm, founded by experienced entrepreneurs with a strong track record in healthtech and large-scale consumer platforms.

Their mission is ambitious: to automate GP-level diagnosis using AI agents, transforming how healthcare is accessed and delivered globally.

This is a hands-on Principal Software Engineer role for someone who wants to operate at the intersection of AI, healthcare, and distributed systems, working directly with real hospitals and clinics to deploy production-grade solutions.

What You'll Work On

  • Deploy and refine enterprise-grade AI systems used in real healthcare environments
  • Build AI-driven patient communication and triage systems

  • Design and operate agent orchestration frameworks with strong guardrails
  • Develop secure healthcare data layers with strict privacy and compliance requirements
  • Implement low-latency voice and video systems for clinical use cases

  • Solve complex problems in distributed systems, scalability, and enterprise integration
  • Work closely with hospitals, clinics, and internal stakeholders to ship real-world solutions

Tech Stack & Focus

  • Primary language: Python (core requirement)

  • AI systems for patient communication and decision support

  • Distributed systems at scale

  • Secure data architectures for healthcare

  • Enterprise integrations and production deployments

  • Flexibility to use other languages where appropriate for senior engineers

What We're Looking For

  • Senior / Principal-level software engineering experience

  • Strong expertise in Python and production systems

  • Experience working with hospitals, clinics, or regulated healthcare environments
  • Proven ability to deploy and maintain enterprise-grade systems

  • Comfort operating in ambiguous, high-impact problem spaces

  • Strong ownership mindset and ability to work autonomously

Why This Role?

  • Direct impact on real healthcare outcomes, not experiments

  • Work on doctor-level AI agents, not superficial automation

  • Backed by top-tier investors with long-term vision

  • High ownership, high trust, and minimal bureaucracy

  • Competitive compensation with meaningful equity upside

Who This Role Is Not For

  • Engineers looking for purely academic or research-only roles

  • Candidates uncomfortable with production responsibility

  • Those preferring narrow scope or rigid processes
